Fast online access to world leading NHS Diabetes Prevention Programme

Residents in the UK can take their health into their own hands and use an online tool, hosted by Diabetes UK, to calculate their risk of developing Type 2 diabetes. The Diabetes UK ‘know your risk’ tool, scores people on their risk of developing Type 2 diabetes. If your risk is moderate or high, you can refer yourself to a local scheme that is part of the Healthier You: NHS Diabetes Prevention Programme and get easy access to free health and lifestyle coaching.

In Dorset, Shropshire and Telford & Wrekin, this service is provided by personalised digital health coaching provider, Liva Healthcare, and in-person educational support from non-profit health provider, Living Well, Taking Control.

Everyone participating in the programme will have a choice between app based, 1-2-1 personal coaching support and in-person support groups. Access to the programme is completely free for users and available without having to go through a healthcare professional.
