The NHS Diabetes
Prevention Programme

The NHS Diabetes Prevention Programme, or NDPP, is a nine-month, evidence-based lifestyle change service available to all those at risk of developing type 2 diabetes.

The programme will help you make positive lifestyle changes, reduce your sugar levels and reduce your risk of developing type 2 diabetes.

You’ll receive personalised support to manage your weight, eat more healthily and be more physically active – which together have been proven to reduce a person’s risk of developing type 2 diabetes.

Someone completing the NHS-funded, nine-month scheme reduces their chances of getting type 2 diabetes by more than a third (37%).

Who can join?

  • People aged 18 or over
  • People who are not pregnant
  • No confirmed diagnosis of type 2 diabetes
  • Those who are motivated to change their lifestyle and can commit to the 9-month programme
  • Registered in an area where Liva is available

Digital one-to-one support

Get started

After downloading the Liva app, you’ll complete the initial assessment survey so we can understand a little more about your current lifestyle.

Book your one-to-one video consultation with your health coach. They will help you prepare for the journey ahead and come up with an action plan to help you achieve your goals over 9 months.

Get focused

The first 12 weeks of the programme focuses on understanding diabetes and how lifestyle can have an effect.

You’ll work towards your action plan with support from your health coach through weekly in-app check-ins (all via video message and text) and community group chat.

Maintain

For the remainder of the programme you will continue to build confidence in maintaining your new, healthy choices over the long term, helped with fortnightly one-to-one and group-based coach check-ins.

You will continue to track and monitor your progress on the Liva app, even after the programme ends.
Specialist and personalised healthy lifestyle support from a health coach over 12 weeks, available via your smartphone or tablet with Liva’s app.
Access the service conveniently through your smartphone, you’ll have support in your pocket. No need to travel or arrange follow-up appointments.
Monitor and track your personal health goals in the Liva app, receive psychological, lifestyle and nutritional advice, and connect with a closed community of other people on the same journey as you, to help you gain confidence and keep up positive lifestyle changes long term.
Connect with a health coach of your choice, with multiple languages spoken to ensure everyone is catered for.

How to join the programme

Step 1

Know your risk

Use the Diabetes UK ‘Know your Risk’ tool, or attend your free NHS Health Check to see if you are eligible to join the programme.
Step 2

Speak to your GP

Ask your GP practice to refer you to the NHS Diabetes Prevention Programme, or NDPP.
Step 3

Await your invitation

Once your GP referral has been received, you’ll receive an email invitation to start.
Step 4

Choose your option

You can choose either the Liva digital one-to-one service or a face-to-face community group service from our partner, Living Well Taking Control
Step 5

Get started

If you choose Liva, we will contact you within five working days to invite you to download the Liva app and complete the sign-up process to get started on your programme.

The only way to access the programme is through a referral from your GP
